Corporation Contracts and Process
Services provided to the Corporation by outside parties require a service agreement and insurance requirements to be met before work can begin. Below are the contract process and agreement templates.
Please email all contracts to ucorp_contracts@csumb.edu
- Obtain a proposal/scope of work "SOW" from the contractor
- More quotes may be required depending on the dollar amount per the Procurement Policy.
- Complete a Service Agreement template and have it signed by the contractor and the authorized signer for the chartstring used for these services.
- Request the Certificate of Insurance as required in section 11 of the agreement.
- If the services are longer than one year, issue a purchase order.
- Email documents to the designated email address above.
- The Executive Director or designee must sign all contracts to be legally binding and fully executed.
- The Department will receive a copy of the fully executed contract. It is the Department's responsibility to send the contract to the contractor.
- Service Agreement 4-16-24 signed by the contractor and department
- Proposal or Scope of Work (Must include a begin and end date and price for the services being provided)
- Certificate of Insurance and additional insureds endorsement as required in section 11 of the Service Agreement
- A purchase order is required if the service lasts more than one year. A purchase order is NOT required if the service will be completed in a year or less. Please refer to the Payment Matrix for more information.

IT-related Service Agreement and requirements
For E&IT purchases, submit an IT Contract Approval ticket with the VPAT and HECVAT attached for accessibility and security review. Once this process is complete, the rest of the purchase may continue.- Auxiliary IT Services Agreement Company 2-15-22 signed by the contractor and department

Departments requesting to waive some or all of the contractor's insurance requirements must submit the forms below. Submitting the forms does not automatically waive the insurance requirements. All requests will be reviewed and approved on a case-by-case basis.
